After downloading one of the forms below, email, fax, or mail them to LCF at the contact information on the form (or on the "Contact Us" link on the left bar).

NOTE:  LCF is affiliated with The National Christian Foundation (NCF), and some of the forms below reference NCF (as well as Fidelity).

Wire Notification Form

• Wire Notification Form – to notify LCF that you are wiring money to your Fund at LCF

Giving Securities

• Letter of Authorization for Stock Transfers – to transfer stock on deposit in your brokerage account. If your shares are held at Computershare, please contact Computershare and request your shares in certificate form. Computershare will send you a certificate, and then use the Certificate Release Request and the Irrevocable Stock Power Form below to transfer your stock certificate to LCF.
  
  If you are transferring stock using your brokerage firm's forms, please use DTC Clearing Number 0226 for National Financial Services and the account number on this Letter of Authorization. Please also send in this Letter of Authorization to our foundation so we will know which fund should receive your contribution.
   
• Master Limited Partnership Stock – If you would like to transfer these types of securities please contact the contributions department at 502-749-4006 prior to the transfer.
   
• Letter of Authorization for Mutual Fund Transfers – to transfer mutual funds to LCF.  Select the institution you're transferring from:
  • Fidelity - this form may also be used to transfer mutual funds if they are held at a firm other than the ones listed. Check with your broker to make sure the mutual funds are transferable. If they are not transferable, contact us at 502-749-4006.

  • Vanguard – Use this prefilled form to transfer Vanguard Mutual Fund Shares to LCF. Please fill out pages 3-6 of the form and fax to Louisville Christian Foundation at 502.749.8580 or send it via regular mail. Once received, the required corporate documents will be completed and submitted to Vanguard. Please allow 2-3 weeks for processing.

• Certificate Release Request – to transfer stock certificate(s) in your physical possession
   
• Irrevocable Stock Power Form (Fidelity Form) – to allow LCF/NCF to facilitate the processing of your certificate(s) in the absence of your endorsement on the certificate(s)

Testamentary giving forms

• Testamentary Gift Information - explains the process for naming LCF as the beneficiary in your will/estate 
   
• Giving Fund Succession Plan Letter of Advisement - allows you to recommend to LCF where your Giving balance is distributed upon your death 
    

Grant recommendation forms

• Domestic Grant Recommendation Form – for grants to U.S.-based charities (though recommending grants is free and fast on our Donor Website)
